Apr - Jun: Grocery Stores & Movie Theaters
Jul - Sep: Gas Stations & Restaurants
Oct - Dec: Hotels, Airlines, Best Buy, Kohls

Please note that you need to activate cash back (sign up) for every quarter and you can do that up 15 days before the quarter begins.

how do we request the wallet sized reminder for these categories? i just happen to get one in the mail for 2011 when i signed up for the card this year.
