I would sure like to believe this is a legitimate offer, but it bothers me a bit that this form is not on the VS site but instead on another site http://www.12lp34.com/ set up 3/8/11.

The phone number given is really VS, and the links on the Thankyou page do redirect to VS. I called to ask them what size the sample is, and reached VS order line but no one seemed to know about the offer.
