    I don't think this is a valid deal. The form is just a PDF, the links at the bottom don't work. I really doubt the folks at Costco will know what to do with it.

      The National Membership offer is for new members and you have to join in store, the pdf is just information regarding the new membership offer. This may also be posted at your Costco location.

      If they didn't receive it, tell them to check at Costco front desk. Front desk will raise a request which takes 2 to 3 weeks to get receive coupon book.

      It was just an image with the information which is available through search engine just the same. The Costco new enrollment bonus is available in store through 6/30/2012.
